Clinical and Research Expertise

Dr. Cherie-Ann Nathan’s clinical interests include head and neck cancer surgery of the  oral, oropharyngeal , laryngeal , hypopharyngeal, nasal, paranasal sinuses and salivary gland cancers. She has a special interest in thyroid and parathyroid diseases. In addition she has a particular interest in voice disorders from both organic and functional causes and has a botox clinic for spasmodic dysphonia patients after ruling out lesions of the vocal cord. Along with neurosurgery she treats cancers of the skull base especially that of the anterior cranial fossa. She heads the Tumor Board, a weekly multi-disciplinary conference to discuss the care of all cancer patients. She has a strong translational research program with many novel projects that have moved from the bench to the bedside (see details below). As a result she has approximately 100 publications and has presented at over a 100 meetings.

Honors and Awards

FWCC’s Otolaryngologist, Cherie- Ann Nathan is a talented surgeon as well as an innovative researcher. Her ideas on molecular analysis of surgical margins have led to the receipt of a large grant from the NCI (National Cancer Institute). A clinical trial is being conducted as a result of these studies, which have put Dr. Nathan’s program at the forefront of molecular targeted therapy for head and neck cancer nationally.

The Cherie-Ann Nathan, M.D. Endowed Professorship in Otolaryngology/Head and Neck Surgery was initiated by a grateful patient who made a generous donations. This professorship will serve as a perpetual tribute to Dr. Nathan’s unwavering devotion to her patients. Donors have given more than $30,000 which is over half the fundraising
goal. Once $60,000 is raised locally, the LSU Health Sciences Foundation will be able to leverage private dollars with $40,000 in matching funds from the Louisiana Board of Regents to establish a $100,000 endowment. Only the interest generated from the funds will be expended leaving the principle to continue Dr. Nathan’s legacy forever.

Research Interest

Pharmacological inhibitors of mTOR. Our laboratory focuses on the use of pharmacologic inhibitors of mammalian target of rapamycin (mTOR) for the treatment of head-and-neck cancer. Inhibition of mTOR by rapamycin analogues inhibits phosphorylation of 4E-BP1 and sequesters eIF4E with 4E-BP1 decreasing translation of mRNAs involved in tumor progression. Our laboratory is determining various adjuvant therapeutic modalities for patients with molecular positive Akt/mTOR positive margins. To determine if rapamycin analogues can potentially be used as adjuvant therapy for patients with mTOR pathway-positive margins, we are elucidating upstream and downstream effectors of the mTOR pathway in head-and-neck squamous cell carcinoma (HNSCC) patients with mTOR overexpressing margins.

Radiation Therapy. Radiotherapy activates cytotoxic cellular mechanisms through the induction of DNA damage and activating pro-apoptotic ASK1/JNK pathway, but at the same time g-radiation further enhances anti-apoptotic PI3K/AKT/mTOR pathway. Rapamycin analogues can effectively inhibit signal transduction downstream from mTOR, attenuating cap-dependent translation of many pro-oncogenic and pro-survival factors. A novel therapeutic approach using a combination of radiation therapy and mTOR inhibition is being investigated for the potential role of mTOR inhibiting agents in the sensitizing of HNSCC tumor to radiation therapy.

PET-imaging. Inoculation of head-and-neck cancer cells subcutaneously into a small surgical incision is used to generate minimal residual disease (MRD) murine model. Functional imaging using 18F deoxy-3”-fluorothymidine (FLT), a proliferation-based tracer, has been emerging as a promising tool to evaluate tumor proliferation activity. Using MRD model we are conducting a comparative study on the usage of 18F fluoro-2-deoxy-D-glucose (FDG)-positron emission tomography (PET) and FLT-PET imaging modalities for the early post-surgical diagnostics of head-and-neck cancer recurrence.

Chemoprevention. We are also interested in chemoprevention studies with natural products. Curcumin, isolated from turmeric, has been implicated as a powerful therapeutic in a variety of human cancers because of its ability to induce apoptosis and is currently undergoing clinical trials for colon, skin, pancreatic, and hematologic cancers, although its effect on oral HNSCC has been limited for in vivo benefit due to low gastrointestinal absorption. The overall goal of this study is to determine whether curcumin can be used as a chemopreventive agent in SCC, and whether local or systemic curcumin treatment affects tumor formation, progression and recurrence. Our goal is to conduct clinical trials with curcumin in HNSCC patients especially those at a high risk of recurrence or even initial malignancy with the goal of decreasing incidence and recurrence rates. Up to 50% of patients develop recurrence of HNSCC therefore preventing any of the steps of neoplastic transformation, from initiation to promotion to progression is a promising strategy.

Research Projects

Project 1, an exploratory biomarker trial of temsirolimus in subjects with newly diagnosed advanced stage squamous cell carcinoma of the head and neck.At the clinical level, the Otolaryngology /Head and Neck Surgery Dept. at LSUHSC, Shreveport with the Feist-Weiller Cancer Center is conducting an exploratory biomarker trial of mTOR inhibitors in subject with newly diagnosed advanced stage HNSCC. The effects of mTOR inhibitor temsirolimus on the phosphorylation state of key proteins in the mTOR pathway in tumor and peripheral blood mononuclear cells (PBMC) of HNSCC patients are being evaluated. Significant inhibition of the mTOR pathway was noted in both tumors and PBMCs of HNSCC with minimal side effects. mTOR inhibitors can potentially be used as adjuvant therapy for patients with MRD and PBMCs are potential surrogate markers in this setting.

Project 2, prognostic markers for head-and-neck cancer recurrence. Multi-institutional trial: molecular analysis of margins. We have determined that the proto-oncogene eIF4E is overexpressed in 100% of HNSCC. A single institutional NIH/NCI funded study showed that eIF4E overexpression in histologically “tumor-free” surgical margins results in a significant decrease in disease free interval compared to patients with eIF4E-negative margins. Now in collaboration with Dr. Koch’s group from Johns Hopkins we are conducting multi-institutional trial to test whether overexpression of the Akt/mTOR/eIF4E pathway in the histologically “tumor-free” mucosal surgical margins of resectable HNSCC patients is an independent prognostic marker for cancer recurrence.

Project 3, radiosensitizing effects of mTOR inhibitors in head-and-neck cancer. Preliminary results of our animal studies suggest that mTOR inhibitors display potential better radiosensitizing properties than conventional radiosensitizing agent cisplatin, and have less toxic side effects.

Project 4, chemoprevention of head-and-neck cancer using natural product (curcumin). We are studying whether curcumin can be used as a chemopreventive agent in head-and-neck squamous cell carcinoma with a goal of decreasing incidence and recurrence rates.
